PENN STATE COLLEGE OF MEDICINE STUDENT HEALTH INSURANCE POLICY

The College of Medicine requires all graduate medical students to have health insurance.

The Fishburn Clinic is available as a primary source of care for students at the College of Medicine. For more information, see Office of Student Health.

ELIGIBILITY

You are eligible to participate in Penn State's Hershey Medical Student Insurance program if you are a registered student at the College of Medicine taking 1 or more credit hours.

The Insurance Company maintains its right to investigate student status and attendance records to verify that the policy eligibility requirements have been met. If, and whenever, the Company discovers that the policy eligibility requirements have not been met, its only obligation is refund of premium.

SPOUSES AND CHILDREN

Eligible students who do enroll may also insure their dependents. Eligible dependents are the spouse and unmarried children under 19 years of age who are not self-supporting. Dependent eligibility expires concurrently with that of the insured student.

Please note: University Health Services does not provide health care services to children; this includes dependent children of students insured through the Hershey Medical Student Accident and Sickness Insurance Plan.

2008-2009 ENROLLMENT DEADLINES

Deadlines for enrollment in this insurance plan are as follows:

Fall 2008 August 31, 2008
Spring 2009 January 20, 2009
Summer 2009 May 26, 2009

The medical insurance premium is billed by the Bursar’s office at the College of Medicine to the student’s account. Please contact the Bursar to decline the health insurance or to complete an application for enrollment.

The completed applications and the payment for the premium will be forwarded to Penn State Student Health Insurance on or before the enrollment deadline listed above. Premium received after these dates will be returned. DATE OF POSTMARK DOES NOT FULFILL THIS REQUIREMENT.
