NOTICE TO INCOMING STUDENTS
Penn State University requires all degree-seeking students to be immunized against measles, mumps, and rubella and to submit this information online. Students are also encouraged to complete an online Health History form. 

You will be able to access the secure website either before you receive your notice to attend FTCAP (using your Friends of Penn State access account ID) or after you have registered for classes (using your Penn State access account ID).

Physical exams are required ONLY for students participating in the INTERCOLLEGIATE ATHLETICS program. This form is not to be used for club sports, intramurals, or other athletic activities (Medical Clearance to Participate in Intercollegiate Athletics UP ONLY-students at campuses other than University Park should contact their campus office for the appropriate form).

These requirements must be met or your ability to register for your second semester at Penn State may be impacted.
