Immunization Reporting Instructions

Print out the sample immunization requirements form.
Print out the sample health history form.
Physical exams are required ONLY for students participating in the INTERCOLLEGIATE ATHLETICS program. This form is not to be used for club sports, intramurals, or other athletic activities (Intercollegiate Athletic Clearance Form).
Complete these sample forms. You may need to obtain some of this information from your health care provider. The sample forms are designed to help you complete the online submission.
Go to our secure website to review our "Notice of Privacy Practices" and enter your immunization and health history information (choose "Forms" from menu bar on left).

NOTE: You will need to enter your Penn State ITS Access Account ID or Friends of Penn State Account ID and your password to access the system.
